JOB TITLE: Social Worker LOCATION: 1155 East Mtn Blvd Wilkes Barre DEPARTMENT:Womens Health WORK SCHEDULE: Days WORK TYPE: Part time .5 FTE 40 hours biweekly payperiod JOB SUMMARY: Care Management facilitates the achievement of client wellness and autonomy through advocacy, assessment, planning, communication, education, resource management and service facilitation across GHS continuum of care, focusing on positive health outcomes for the individual in a resource and cost effective manner. Applies national standards of care to all patients. Utilizing the medical-social model, the Care Manager coordinates interdisciplinary care of a select caseload of patients and helps ensure that patients receive care in the sequence and setting that is most appropriate in assisting patients to achieve their highest level of function and return to the least restrictive environment. Provides appropriate psychosocial interventions to address personal or emotional problems that may affect the patients' pregnancy outcome. Responsible for ensuring the very best patient/family experience by optimizing all customer interaction. Foster a positive work environment through mutual support and cooperation and adheres to GHS and unit specific Service Standards. Accountable to the Manager, Care Management. EDUCATION AND/OR EXPERIENCE: Master's degree in Social Work (MSW) from an accredited university required. Ability to obtain Social Work licensure and Care Management certification within five years of employment in Care Management Department required.
