J OB SUMMARY:
This position supports the Infectious Diseases providers to care for patients with a wide variety of serious non-orthopedic infections such as endocarditis, brain abscess, and intra-abdominal abscess. At any given time there are 30-40 of these types of patients per week on home intravenous antibiotics. This requires regular and careful monitoring of labs and potential adverse events. Frequent interaction with a variety of home health agencies and pharmacies is needed to provide safe and effective care. The person in this role provides this time-consuming but vital coordination and oversight. This person also helps to evaluate and educate many of our 700 HIV patients in the ambulatory care setting. This interaction is essential because the nurse serves as the first point of contact when our HIV patients call/contact the ID office with concern or questions; the prior interactions in clinic help the patients feel comfortable with the nurse and trust in his/her care. Because ID does not have a "home" clinic site (providers are spread out at multiple sites), the person in this position responds to patient concerns instead of the nurses and medical assistants in the clinics. These staff are not familiar with our patient's unique issues and are not comfortable addressing their questions or concerns.
Bachelor's degree in Nursing or Master's degree in Nursing required.
3 years clinical experience in specialty area required.
Currently licensed to practice as a registered nurse by Pennsylvania Board of Nurse Examiners required.
KNOWLEDGE, SKILLS, & ABILITIES:
Strong communications skills both oral and written required.
Strong organizational skills required.
Ability to analyze data reports on patient outcomes required.
In a team setting, attends selected ambulatory care sessions each week, working directly with HIV-infected persons and other Infectious Diseases patients to assess patient problems, promote good health, provide patient/family education, and contribute to the formulation and implementation of the treatment plan. Follows up with patients to evaluate effectiveness.
Coordinates the outpatient intravenous antibiotic care plan for Infectious Diseases patients after the transition from inpatient to outpatient care; reconciles medications, assesses laboratory test results, serves as a liaison with home health agencies and pharmacies, and assists with troubleshooting for antibiotic side effects and central line-related complications, collaborating with providers.
Serves as a primary point of contact for the Infectious Diseases and HIV-infected patients for concerns, questions, and test results, communicating with patients via telephone, the patient portal, and other modalities; keeps providers and other team members informed regarding relevant clinical changes and needs.
Participates in team meetings, conferences, and QI activities, working effectively within the team setting with the unified goal of optimal patient care and patient satisfaction.
Completes paperwork/faxes for patient orders, often related to the outpatient intravenous antibiotic program; also completes forms for insurance authorizations for ID and HIV patient medications, diagnostic tests, FMLA, etc., with assistance from other team members as needed.
Experience as a case manager, in discharge planning or insurance issues preferred.
Appropriate certification for specialty area preferred.
Proficiency in Microsoft Office preferred.
Motivational interviewing skills preferred.
