SUMMARY OF POSITION:

Working in a team setting, responsible to coordinate the professional care of Infectious Diseases patients. 

RESPONSIBILITIES:

ESSENTIAL FUNCTIONS: The percentage of time spent performing essential functions is 97%. Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

1.In a team setting, attends selected ambulatory care sessions each week, working directly with HIV-infected persons and other Infectious Diseases patients to assess and promote good health, provide patient/family education, and contribute to the formulation and implementation of the treatment plan.

2.Coordinates the outpatient intravenous antibiotic care plan for Infectious Diseases patients after the transition from inpatient to outpatient care; reconciles medications, assesses laboratory test results, serves as a liaison with home health agencies and pharmacies, and assists with troubleshooting for antibiotic side effects and line-related complications, with provider oversight.

3.Serves as a primary point of contact for the Infectious Diseases patients for concerns, questions, and test results, communicating with patients via telephone, the patient portal, and other modalities; keeps providers and other team members as appropriate updated regarding clinical changes.

4.Participates in team meetings, conferences, and QI activities, working effectively within a team setting with the unified goal of optimal patient care and patient satisfaction; additional team members include nurses, pharmacists, case managers, counselors, dieticians, administrative assistants, advanced practice providers, physicians, others.

5.Completes paperwork/faxes for patient orders, often related to the outpatient intravenous antibiotic program; also completes periodic paperwork for insurance authorizations for patient medications and diagnostic tests, FMLA, other, with assistance from other team members as needed.

NON-ESSENTIAL FUNCTIONS: The percentage of time spent performing secondary functions is 3%. The following duties are considered secondary to the essential duties listed above:

MINIMUM QUALIFICATIONS:

Bachelor's degree in nursing required. 

Three (3) years of nursing experience required. 

License to practice as a registered nurse in Pennsylvania required.

KNOWLEDGE, SKILL & ABILITIES:

Motivation to provide high quality patient care required.

Strong communication skills both oral and written required.

Excellent organizational skills required.

Ability to analyze patient data reports/lab reports required.

Effective teamwork required.

PREFERRED QUALIFICATIONS:

Experience in Infectious Diseases and/or HIV care preferred.

Proficiency in computer skills such as Microsoft Office preferred.
