PRINCIPAL DUTIES AND RESPONSIBILITIES:
Maintains positive relationships and collaborates with external stakeholders in order to maximize outcomes and regulatory compliance
Promotes a professional practice environment that fosters teamwork and collaboration among team members. Ensures staff growth and development through mentoring programs and identifying and addressing gaps in competencies
Identifies areas of potential compliance risk and develops strategies to mitigate risk in collaboration with the Compliance Officer and staff
Develops the processes, policies, and procedures impacting all daily operations of case management and social work at assigned locations
Develops systems and processes to effectively manage collaborative relationships with internal stakeholders and external post-acute providers
Prepares and reviews annual performance appraisals with employees.
Facilitates regulatory focused committees as required
Develops processes for the accurate capture of data related to transition planning and denial management.
In collaboration with the key internal stakeholders, develops and implements processes, policies, and procedures to effectively and efficiently manage the transition of patients in accordance with state, federal and payer standards and regulations
Ensures timely completion of event reports related to case management operations
Meets with patient relations and patient/families as necessary to address patient concerns related to case management/transition planning.
Analyzes data and recognizes trends where opportunities may exist. Performs as an effective team member as demonstrated by interactions within department and with ancillary departments impacted through the case management process.
Provides direct management functions to include input regarding hiring,
Demonstrates superior knowledge of regulatory requirements specific to case management, utilization management, EMTALA, and transition planning.
Serves as an expert care management resource for staff and Christiana Care leadership
Collaborates with other departments to ensure best practice standards and promotes high quality care with excellent outcomes.
Maintains a collaborative professional relationship with clinicians and administrative staff necessary to develop, implement, and evaluate successful process outcomes
Provides direction and mentoring to the management team. Assists with staff positive coaching/disciplinary actions as necessary
Develops revenue/expense, capital equipment, renovation, and manpower operating budgets for assigned cost centers; monitors compliance with budget on a regular basis.
Performs assigned work safely, adhering to established departmental safety rules and practices; reports to the supervisor, in a timely fashion, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients or visitors.
Performs other related duties as directed.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Bachelor of Science degree in Nursing with professional licensure required
Master's degree in Nursing, Healthcare Administration, Business, Health Care Delivery Science or related field required
Minimum 5 years of experience in Utilization Management, Case Management, Acute Care and/or Ambulatory Care required
Minimum 3 years of supervisory/management experience
Certified Case Manager (CCM), Accredited Case Manager (ACM) or equivalent certification preferred
Demonstrated capability in development execution of strategic initiatives
Demonstrated track record of improving care outcomes
Demonstrated capability in setting and delivering program goals
Active Delaware license. CCM, ACM or equivalent certification required within 2 years
