Position Information:
The Population Health Nurse role is designed to improve quality, reduce cost, promote optimal well-being across a continuum of care and is an integral part an interdisciplinary team. The Population Health Nurse is responsible and accountable for clinical expertise for specific patient populations for individuals with chronic conditions; potentially complex medical, mental health, and psychosocial issues. Utilizes health coaching techniques to perform disease management, assessment of disease states and referral to appropriate levels of care. Focus on identification of patients appropriate for Annual Wellness Visits and execute guided processes to complete the visit. This role requires diverse job functions to meet specific contractual and program related requirements to achieve optimal clinical outcomes. Performs related duties as assigned.

Experience Requirements:
3 Years Healthcare - Ambulatory Care Preferred - And/Or Clinical - Home or Community Health Preferred

Education Required:
Graduate Nursing Required - And Bachelor's Degree Nursing Preferred

Licensure/Certification Requirements:
Registered Nurse Required

Hours of Work:
Monday-Friday

Weekend Requirements:
No Weekends
