Admissions and Transition Coordinator - Correctional Care
Job SummaryUnder supervision of the Program Manager, the Admission and Transition Coordinator of the Restoration and Transition Unit will coordinate all aspects of the assessment and admission process for inpatient care for the Jail Based Restoration Program. In addition, the Admission and Transition Coordinator will lead and coordinate transition efforts to develop wrap around services for patients associated with competency restoration to ensure access to care and equitable outcomes.
